Potholes, Other Street Repair

Hello, I live at 4209 E Sheffield Dr. The road at the end of my driveway is becoming deteriorated and forming potholes. the road is at a down hill slop so recently when we have been getting these really hard rains its worsened the issue quite a bit. Over the years as you can see in the photo ive laid down concrete to attempt a patch job myself. Now it is beyond my repair im afraid. Any help on this repair would be greatly appricated.
